Most cancers arise from <strong>somatic<\/strong> mutations accumulated during life \u2014 exposure to tobacco, UV radiation, chronic infection, obesity, age \u2014 but approximately 5-10% are <strong>hereditary<\/strong>, driven by germline variants inherited from a parent in cancer-predisposing genes such as BRCA1\/2, TP53, MMR, APC, RB1.<\/p><p><strong>Genetic testing in cancer<\/strong> serves three purposes: (1) <strong>diagnostic<\/strong> \u2014 identifying hereditary cancer syndromes that change surveillance and family management, (2) <strong>predictive<\/strong> \u2014 risk stratification of healthy at-risk relatives, and (3) <strong>therapy-guiding<\/strong> \u2014 identifying targets for molecular therapy (e.g. PARP inhibitors in BRCA mutant tumors, EGFR\/ALK in lung cancer, mismatch repair status guiding immunotherapy).
